Certification Update/Duplicate Request

A complete application should include the following:

1. Complete form ISBE 73-71 Certification Update/Duplicate Certificate Request

Please complete the following option(s) that apply to you:

• Section I-Notice of change of address
• Section II- Name change
• Section III-A degree change (attach an official transcript in the original sealed envelope to update our records)
• Section IV-Request for a copy of your certificate(s).

2. A $8.00 fee (per certificate) money order, personnel check, or certified check made payable to ROE 17. (This applies to Section II or IV).

3. Sign and date the form and return to ROE 17.
